After contacting me by phone or email, I will work with you to assess your elder's needs as the first step in creating a personalized Action Plan.

You may also contact me by email: maria@ElderCareAcrossTheMiles.com with a brief description of where your loved one lives in Sonoma County, your relationship to him or her, and your location — all we need to start the conversation.

Needs Assessment

Once we have had our first conversation about your elder's situation, please download and fill out what you can on the Needs Assessment Form (PDF). There is a charge of $50 for completing this assessment once we have worked on it together. We can expect the Needs Assessment will change as we develop an Action Plan. There is no additional charge for such changes unless there is a major change in the elder's health or living situation that may require a new Needs Assessment.
