Three Easy Steps:
- Assess your elder’s current needs for diet and meals, housekeeping help, routine appointments with doctors and dentist, transportation, hearing and sight aids, hired in-home caregivers, foot care, safety at home, mobility, exercise, social outlets and religious participation.
- Create an action plan listing specific services, programs and other resources to meet those needs, along with estimated costs and alternatives. Resources may include agencies and professionals.
- Coordinate delivery of services with your input and oversight. Make adjustments until the quality and level of care are met to your and your elder’s satisfaction.
